COMPLAINTS

If you believe your privacy rights have been violated, you may file a complaint with our Office Manager at the address above or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

OTHER USES OF YOUR HEALTH INFORMATION

Any use or disclosure not covered in this notice or required by law will be made only with your written authorization. You may revoke this authorization at any time in writing.
